Disputes forms

The disputes panel regulations provide forms to be used during the dispute process. This page provides the forms that must be used and other forms that are helpful to use.

  1. Dispute notice
    This form is a suggested form to use. It's the notice that the applicant gives to the respondent at the start of the dispute process. If the operator is not a party, the applicant gives them a copy.
  2. Reply to dispute notice
    This is a suggested form for the respondent to use if they wish to respond to the dispute notice.
  3. Request to witness to attend hearing of disputes panel
    If any party wishes to have a witness appear at the hearing they may use this form to request the witness' attendance.
  4. Application to disputes panel
    If a party wishes to make an application to the disputes panel about certain matters they must use this form.
